Before Your First Appointment

In an attempt to make your first appointment an easy, more enjoyable experience, we have made it possible to get all of your information needed available to you at your home.

 

We request that you print out all of the required intake forms, giving careful attention to confidentiality and billing polices. Please bring these with you to your first appointment.

 

Rates: Some insurances are accepted. Many of our patients also qualify for out of network benefits if we are not listed as a preferred provider with an insurance carrier. Please inquire about our sliding scale for income eligible clients not using insurance.

 

 

How to Manage your Insurance

If you would like to use insurance, please contact your insurance company prior to your first appointment to understand your eligibility and reimbursement benefits.

 

Here are a number of relevant questions to ask your provider to help you better understand your benefits. Have your insurance identification card ready and the name of your counselor. Call us for our federal tax identification number.

 

With this information you may want to ask the following:

 

Is this provider in my network? If yes, what is my co-payment amount?

If not, do I have out-of-network benefits?

What is my deductible?

What percentage will the insurance company pay and what percentage do I have to pay?

Where do I send the receipts for service?
